A Brief Overview
Serves as a contributing member on the Behavioral Health team; provides screening and assists with patient disposition plans that present with a health crisis. Helps Behavioral Health patients connect with resources that allow them to manage mental crisis as they arise; improve overall health and potentially avoid hospital encounters. Assists with posting discharge care and connecting patients to community resources and supports for individuals with complex circumstances.

Essential Functions

Provides brief interventions and psychoeducation to support patient wellness and recovery.
Works in a highly collaboratively manner with therapy clients' assigned case managers, and other service providers, within and outside facility.
Assists in developing measurable individualized therapy goals and objectives.
Serves as the primary contact for all payors regarding utilization review and management issues.
Delivers support of clinical integration needs of other clinical services, and collaboration with medical director leadership.
Maintains a patient first philosophy and engages in service recovery when necessary.
Participates in clinical huddles when working with medical & behavioral health providers to review the appointment schedule for the day and determine which patients will need visits.
Ensures that documentation compliance requirements are met consistently.

Education Qualifications

Bachelor's Degree from an accredited college or university in Social Work or related field Required

Experience Qualifications

Experience working in a clinic or hospital setting Preferred

Licenses and Certifications

BLS - Basic Life Support through the American Heart Association Upon Hire Required and
Safety-Care Specialist within 60 Days Required
