A Day with CenterWell• See 12-15 patients per day, allowing for personalized patient interactions
• Play a crucial role in a value-based care model, boosting patient's quality of life
• Handle a diverse caseload from preventive care to chronic conditions.
• Dedicate 100% of your time to direct patient care, including patient-facing time and associated administrative tasks
• Active, unrestricted NP/PA license in the state
• Current NP or PA certification
• Current DEA license or the ability to obtain one
• Passion for enhancing consumer experiences within an organization
• This role is patient-facing and part of Humana/Senior Bridge's TB screening program. TB screening will be required.
Preferred Skills• Familiarity with Value-Based Care model
• 2-5 years experience with geriatrics patients in a primary care setting
• Autonomy in chronic disease management
• Experience in outpatient senior care
• Knowledge of HEDIS measures
